Absolute Wellness Center offers sports medicine, chiropractic medicine, naturopathic medicine, physical rehabilitation, acupuncture, and therapeutic massage. We provide only the highest quality of patient care and our focus is on empowering our patients in their healing process so they are up and running with fewer visits and greater confidence.
Appointments or Questions
If you would like to make an appointment, please call our office at 541-484-5777. We are here to answer any questions you have and address any concerns you may have. Our staff, our providers, and Dr. Herb are available and happy to talk you. If you are unsure of what steps to take, an initial exam with one of our physicians is often most helpful so you get a proper diagnosis and discussion of what treatment options are available to you, whether those options are at our clinic, another office or specialist in town, or self-care you can do from home.

First time patients are welcome to arrive 20-30 minutes prior to their scheduled appointment to fill out their paperwork in the comfort of our waiting area. For those of you who prefer to fill out paperwork at home, please use the links below to download and print our forms. New Patient Health Packet – (Required for ALL New Patients)
This packet includes a New Patient Health History Form, our Privacy Notice, and a Patient Authorization Form. If you have any questions at all, just give us a call or ask us when you come in for your appointment.
